ELIGIBILITY INFORMATION

The Ozanam Charitable Pharmacy serves individuals in Mobile, Baldwin and Escambia counties in southwest Alabama.  To use our services, you must be pre-certified. 

Who is eligible?

Eligibility depends greatly on the ability to document the need.  Most applicants are approved for a three to six-month period.  In some circumstances, approval may be given for only one month until you can bring in more extensive documentation.  Home-bound clients may apply through a relative or designated caregiver.  This person should bring a letter from you that gives permission to apply for and/or pick-up medications.  Identification and Social Security numbers of your designated persons will be noted in your file and will be checked at the pharmacy window when medications are picked-up. 

When your certification expires, you will need to bring all original financial information and be re-screened by your case worker.  You should not wait until the day your certification is due to expire.  No new written prescriptions are needed if refills are left on your pharmacy record. 

You may also be eligible for the Access Your Rx Program that helps you get additional medications through pharmaceutical manufacturers' Patient Assistance Programs .

Patients of Mobile County Health Department and Franklin Memorial Clinics will be certified by social workers at those clinics.

What to bring

Social Security Number: Your own and those of other people living with you.
Proof of income: Includes salaries, pensions, assistance from other agencies, insurance and income of any other persons in your household.
Proof of expenses: Not be limited to items such as rent, mortgage notes, utilities, medical expenses that are not covered by any insurance plan, monthly pharmacy expenses, medical equipment rentals, recent home repairs and any unusual event or situation that has impacted the household income in recent months. 
Be prepared: To review your health history and to let the pharmacist know of any changes since your last visit.
